FACT (Families and Agencies Coming Together)
Holly Bayer and Tracie Johnson are the FACT (Families and Agencies Coming Together) representatives who help families and students in the Albany School District seeking access to health and social services. They work with pregnant and parenting teens, setting up HUD Housing, Oregon Health Plan paperwork and many other services that may be helpful. They can be reached at 541-924-3720.

Your Child or Teen Can Have Health Coverage!
• We know an education is the best gift we can give our children. We also know that healthy kids learn better and healthy kids start with health coverage that their families can afford.
• Recently your child or teen
received a school flyer about the Healthy Kids program. Healthy Kids is Oregon’s
no-cost and low-cost health coverage program for uninsured kids and teens up to
19 years of age.
• Healthy Kids is
comprehensive coverage offering medical, dental, vision, prescription, and
behavioral health coverage.
• No family makes too much
money for Healthy Kids. Family income will determine whether the child qualifies
for the no-cost, low-cost or full-cost option. For example, a family of four
that earns as much as $69,383 a year may qualify for low-cost coverage.

• Sabemos que la educación es el mejor obsequio que les podemos dar a nuestros hijos. También sabemos que los niños sanos tienen un mejor aprovechamiento académico, y para que los niños estén sanos necesitan una cobertura de salud que esté al alcance del bolsillo de las familias.
• Hace poco su hijo/a recibió
un folleto sobre el programa de Healthy Kids. Healthy Kids es un nuevo programa
en Oregón, que ofrece cobertura sin costo, de bajo costo y de costo completo
para niños y adolescentes hasta 18 años de edad.
• Healthy Kids cubre todas
las necesidades médicas, servicios dentales, de la vista, recetas y de la salud
mental o conductual.
• Para Healthy Kids no hay
familia que gane demasiado dinero. El ingreso familiar es lo que define la
opción a la que su niño o adolescente tiene derecho: sin costo, de bajo costo o
de costo completo. Por ejemplo, una familia de cuatro personas que tenga un
ingreso de hasta $69,383 al año puede cumplir con los requisitos de la opción de
bajo costo.
