Greater Albany Public Schools Health Services Team

The GAPS Health Services Team was established in July 2021 to collaborate efforts and create a comprehensive student support system for health services. The Health Services Team comprises all our district nurses, counselors, and Family and Community Together (FACT) team. These individuals work together to provide equitable health services, mental and physical, to all students at GAPS.

District Nurses

Our school nurses collaborate with parents, school staff, and community health care professionals to meet each student’s specific needs during the school day.

FACT Office

The Families and Community Together (FACT) team links schools, students, and families with available community services and resources in the Albany area.

Counseling Office

Our school counselors help students form healthy goals, mindsets and behaviors, and provide guidance for a career after high school.

Immunization Information

Oregon law requires all children attending school, preschool, or daycare be up-to-date on their immunizations, unless they have a medical or non-medical exemption on file. Follow the link above for more information.
