Please be sure to select 2025-26 as the school year for all new K-12 students intending to start in Fall 2025.

Verification for returning students begins on July 1st.

Step 1: Online Registration and Verification

Once you complete the registration process in step 1, you will be directed back to this website to finish completing any needed forms. Please read through the following questions to determine which forms apply for your student(s). You can choose to print the forms you need once you have filled them out or save them to your device. If you print, please deliver them to your child’s school office. If you prefer to email them, please send to: district.nurse@albany.k12.or.us; or mail to 701 19th Ave SE, Albany, OR 97322.

1) Will your child need medication(s) while at school? If yes, please complete the appropriate forms below:

2) If you would like your student’s District Nurse to contact their physician to obtain orders and related health information please complete the fillable Release of information form below:

Step 3: Optional – Pay for Registration Fees

Pay Registration Fees (Grades 6–12)

• To pay Registration Fees go to: https://or-albany.intouchreceipting.com/
• To log into your student’s Activities/Fee account, you will need the StudentID. Contact your school’s Office Staff to get your StudentID number.
• The district does not charge a transaction fee.

Additional information

Class and athletic fees will be paid after completing registration in step 1.

Middle School 

Complete online registration in Step 1, fill out school forms, receive locker and schedule, and pay registration fees if not paid online (optional).

Elementary School 

Enrollment Requirements
Complete online registration, provide proof of address (Kindergarten and New Students only), and Fill out After School Planning form and Transportation Permission slips.

High School 

Complete online registration, fill out school forms, receive locker and schedule and pay registration fees if not paid online (optional).
